Current Reservoir students, please complete the Current Student Transcript Request Form.  NOTE: This is form is NOT used for college transcript requests.  All college transcript requests will be made through Naviance.  Please see your counselor for more information. 

Former students who graduated from Reservoir within the past 5 years (class of 2019-2023 only) and need a transcript should complete the Former Student Transcript Request Form.  $5 payment is required before submitting the form.  

Students who have graduated more than 5 years ago (prior to 2019), Please complete this FORM instead.  These transcript requests are processed by the county's transcript office, not by the Reservoir HS Registrar.
